Tranquility Grove is a residential program. Our address is shared with admitted residents and their families.
Admission Process
Complete Referral Form
Either in office, by phone, via email, or online. Submit important documents, such as insurance, relevant medical records, and necessary financial information.
Complete the Screening Process
With one of our trained staff. This determines your eligibility for 3.1 Residential Treatment Services.
Complete the Assessment
With one of our Assessors. A comprehensive clinical assessment will be performed to determine the most suitable treatment recommendations and programming.
Complete Admissions
By coordinating arrival and entry into the home.
